Clinical Significance:

There is no ideal method for assessment of steroid hormones. Blood, saliva,and urine each have their advantages and disadvantages (see Comparison of blood, urine, and saliva tests); however, practitioners across the country have relied on our hormone tests to yield useful clinical information in the diagnosis and treatment of hormone-related problems for over 15 years.

The 24-hour urine sample may be more or less challenging to collect compared with blood and saliva samples; however, we feel it is more accurate because it smooths the well-known circadian fluctuation in hormone levels. A small study was performed recently at Meridian Valley Laboratory in which eight subjects ingested asmall amount of pregnenolone daily for 28 days. Comparison of hormone profiles taken before the start of the study and on the 28th day indicated that as expected, supplemental pregnenolone can be metabolized to any ofits potential daughters in the steroid family tree although the results were unpredictable in the sense that no two individuals metabolized the supplement in quite the same fashion. This supports the concept that urinaryhormone levels reflect normal physiologic processes. The simultaneous measurement of all major hormone levels was also advantageous for the purpose of the study.

A recent concept in hormone-related cancer prevention is the measurement of the ratio of two estrone metabolites: 2-hydroxyestrone and 16a-hydroxyestrone. A decrease in the 2/16a ratio is associated with an increased risk of breast and cervical cancer (read more about the 2/16 Ratio Test). The 2/16a ratio is not included in the standard hormone assay but is available as an add-on.

We have also noted that 24-hour urinary estrogens can be a sensitive monitor of liver detoxification capability. Elevated urinary estrogens in normally-cycling women may indicate a history of exposure to liver stresses such as excessive environmental organic chemicals. Interventions intended to improve liver function result in a gradual normalization of the abnormal estrogen levels. Thus, measurement of urinary estrogens can give insight into other aspects of physiology. This phenomenon is also noted in peri- or post-menopausal women who have previously taken Premarin, and have switched to triple-estrogen replacement with less-than-optimal symptom relief.
