Payment
To better understand our payment policies you can read about the following two types of accounts, BILL DOCTOR and PATIENT PREPAY. Meridian Valley Lab does not accept insurance.
I. Bill Doctor
You pay your physician directly for the test. Your provider is then responsible for us (MVL).
II. Patient Prepay
You’re responsible for paying us (MVL). Please send your payment (ie credit card information) with your lab test. We accept checks, money orders, Visa/MasterCard/Discover/AMEX.
Refund Policy
- There will be 85% refund within 120 days for unused kits. 15% handling fee will be charged; if we shipped the kit directly to the patient the shipping fee is non-refundable. No Refund after 120 days.
- There will be 70% refund for test submitted and if testing has not been initiated. 30% shipping and handling fee will be charged.
- There is NO refund once the testing has been initiated.
Releasing the Reports
- The reports will be faxed (per ordering doctor’s account set-up) and mailed directly to the ordering doctor’s office.
- We do not e-mail the reports, it is against HIPAA Law.
- Meridian Valley Laboratory releases results only to patients who have paid MVL for their testing, if they sign a Release Form. We are unable to release results to you if you pay your doctor directly.
